Providence woman dies in head-on crash in Cache County

CACHE COUNTY, Utah, Sept. 16, 2017 (Gephardt Daily) — A Providence woman died Saturday afternoon in a crash on State Route 218 near Smithfield.

According to a Utah Highway Patrol news release, the collision happened at 3:40 p.m. at mile marker 6.

Samantha Perkes, 28, was driving a Pontiac Grand Am westbound on SR-218, with her 7- and 4-year-old daughters in the car.

A 15-year-old Cache County male was driving a Ford Fusion eastbound on SR-218. He was alone in the vehicle.

The Ford veered into the westbound lane and caused a vehicle in front of the Pontiac to swerve out of the lane to avoid a collision, the release states. The Ford hit the Pontiac head on.

Perkes was killed instantly. Her daughters, who were properly restrained in the back seat, had minor injuries. They were transported to Cache Valley Hospital in North Logan, where they were treated and released.

The driver of the Ford suffered head injuries in the crash. He was transported to Cache Valley Hospital and then flown by Life Flight to Primary Children’s Hospital in Salt Lake City.

The vehicle that swerved was not involved in the crash, the release says.
