Health Department violations close McDonald’s, Thai restaurant in downtown Salt Lake City

SALT LAKE COUNTY, Utah, Oct. 30, 2017 (Gephardt Daily) — Two near westside Salt Lake City eateries and one Murray catering business were closed by the County Health Department last week for violations that ranged from dust-covered food equipment to dead cockroaches.

Closed on Oct. 25 by the Salt Lake County Health Department were a downtown McDonald’s at 210 W. 500 South (five violations); Main Events Catering of Murray, 167 W. 4500 South (13 violations); and Sala Thai Kitchen, 679 S. 200 West (25 violations).

At McDonald’s, all violations were related to staying open during a dusty, dirty renovation. According to the report:

  • This establishment is under construction and food cannot be prepared in a sanitary manner.
  • Food equipment is coated in dust.
  • The roof has large holes exposed to the elements.
  • Flooring has been removed and dirt is exposed.
  • Construction work has not been properly contained to protect food and food service items.

At Main Events Catering, violations were related to the business’s failure to apply for a license and meet home-business qualifications.

  • This establishment is operating without a valid Food Establishment Permit from the Salt Lake County Health Department.
  • Motorcycles are located in the kitchen area near food and equipment storage.
  • Items that are unnecessary to the operation of the food establishment are stored on the premises (clothing, personal care items).
  • Dog bowls and food are located in the kitchen.
  • There are living quarters connected to the kitchen.
  • There is no certified food safety manager employed at the establishment.
  • Not all floor/wall junctures are covered and sealed.
  • The toilet room doors are not self-closing.
  • Food is being stored in non-food grade containers.
  • Various ceiling surfaces are not smooth, durable, easily cleanable and non-absorbent.
  • Various floor surfaces are not smooth, durable, easily cleanable and non-absorbent.
  • Various wall surfaces are not smooth, durable, easily cleanable and non-absorbent.
  • Wall surfaces behind the cook area (are) not durable.

The Sala Thai Kitchen had more than two dozen issues, most related to cleanliness and food safety.

  • There is insufficient capacity in the facility to hold potentially hazardous foods at 41 degrees Fahrenheit or below.
  • Food is being thawed in the hand-washing sink.
  • Dead cockroaches have not been removed from light fixtures.
  • Exterior surfaces of cooking equipment are dirty (back and sides of the grill/fryer).
  • The person in charge is not ensuring that persons unnecessary to the food establishment operation are not allowed in the food preparation, food storage, or ware-washing areas (children in the service area).
  • Potentially hazardous food is thawing at room temperature.
  • The 3-compartment sink faucet is leaking.
  • A water heater is leaking into a can.
  • Single-use items are being re-used.
  • Walls are dirty in various areas.
  • The floor is dirty beneath cooking equipment.
  • The floor is dirty in various areas.
  • Salads were prepared with bare hands.
  • Potentially hazardous food is being cooled in covered containers.
  • The person in charge is unable to demonstrate adequate knowledge of proper hot and cold holding temperatures for potentially hazardous foods.
  • Food safety manager certification is expired.
  • Bean sprouts are being held out of temperature control at 60-71 degrees on a counter.
  • Cooked-chicken skewers are being held at 51 degrees in a reach-in cooler.
  • Tofu is being held at 47 degrees in the make table.
  • The reach-in is unable to hold potentially hazardous foods at or below 41 degrees.
  • One make table is being held at 57 degrees. Another is at 47 degrees.
  • An employee did not wash hands between clean and dirty tasks.
  • An employee did not wash hands before putting on gloves.
  • Noodles are stored on raw egg shells in the reach-in.
  • Raw beef is stored in contact with limes.
