WEST JORDAN, Utah, Feb. 19, 2016 (Gephardt Daily) — Police have identified a man crushed in an industrial accident after a large sheet of steel fell on him Friday morning.

According West Jordan Police Department, Alfonso David-Grajeda, 37, of Orem was killed just before 7 a.m. at SME Steel Contractors in West Jordan.

Officer Scott List of the West Jordan Police Department described what happened.

“They are an industrial steel company, and they had a male employee who was working on, in my estimation, about a 40-foot in length structure, about 10 feet wide,” List said.

“He was cutting it, and at some point he cut through enough that it fell on him. He was underneath it, and it landed on his head and neck area. He did succumb to those injuries.”

List said the section of steel weighed several thousand pounds. OSHA will be investigating, he said.

“I’m not aware at this point of any safety precautions that could have been taken, or weren’t, or were,” List said.

No one else was injured. The man’s coworkers removed the steel, but he did not respond to first aid attempts, List said.

OSHA will head the investigation, List said.

“As far as the West Jordan Police Department, it appears to us to be an accident,” List said. “We don’t see any evidence leading to any other conclusion.”
