TOOELE, Utah, Sept. 28, 2018 (Gephardt Daily) — Authorities have identified the woman killed in a head-on collision in Tooele Thursday afternoon.

The deceased is Heather Slowik, 47, from Stockton, Utah, according to a Utah Highway Patrol news release.

Two other people were airlifted to an area hospital in critical condition.

The accident happened mid-afternoon when the driver of a red Jeep Liberty, southbound on State Route 36 near mile marker 66, attempted to make a lane change and struck a Hyundai Elantra.

The Jeep then crossed into the northbound lanes, striking a Honda Civic head on.

“The driver of the Honda, who was the sole occupant, suffered fatal injuries from the accident,” the news release said.

“The driver and passenger in the Jeep were airlifted from the scene to the hospital. Their conditions are unknown,” said the UHP statement.

Traffic was blocked on SR-36 and on nearby Interstate 80 while the crash was investigated and cleared. Cars were lined up for miles in both directions from Exit 99 for hours.
