Arrest made in Hurricane auto-pedestrian fatality

HURRICANE, Utah, June 2, 2026 (Gephardt Daily) — Police have arrested the alleged driver in a fatal May auto-pedestrian crash.

Tammey Lynn Crouse, 66, was hit May 5 while in a crosswalk at 700 W. State St. and died from her injuries.

Investigation determined that Crouse was crossing State Street walking northbound in a marked crosswalk when she was struck by a vehicle driven by 68-year-old Wayne Allan Levitz, Hurricane City police said Monday evening.

“Levitz was travelling southbound on 700 West and was making a left turn eastbound onto State Street when the collision occurred.”

Police said following the completion of the investigation and consultation with the Washington County Attorney’s Office, “probable cause was established to charge Levitz with negligent homicide. Levitz has been arrested and booked into the Washington County Jail on that charge.”

Jail records show Levitz, of Hurricane, was booked at 3:50 p.m. Monday, the charge listed as a class A misdemeanor. He was later released on his own recognisance.
