Driver killed in head-on crash with semi in Duchesne County

DUCHESNE COUNTY, Utah, Oct. 19, 2018 (Gephardt Daily) — A head-on crash Thursday night between a passenger car and a semi pulling double tankers ended with the driver of the car dead on impact and the tractor bursting into flames.

The fatal collision happened just after 11 p.m. on State Route 40 near mile marker 80.5, a Utah Highway Patrol news release says.

A passenger car was westbound on SR-40, and a semi was heading east at the same location.

“The passenger car suddenly drove into the eastbound lane and collided with the semi truck head-on,” the news release states. “The semi continued on for a short distance before being able to stop. The tractor then ignited into flames and completely burned.”

The tankers it was hauling were undamaged.

Two people were in the passenger car. The driver of the car, identified as Jacob A. Kosowsky, 21, of Boca-Raton, Florida, was killed on impact. The front passenger was transported to Uintah Basin Medical Center with injuries that were not life-threatening.

The driver of the semi truck was not badly injured and was not transported to a hospital, according to the news release.
