Davis County now offering Pfizer vaccine to ages 12 and older

DAVIS COUNTY, Utah, May 13, 201 (Gephardt Daily) — Davis County is now offering vaccinations to anyone ages 12 and older, officials announced Thursday.

“Appointments for the Pfizer vaccine are now available to be made for anyone 12 years of age and older at the Legacy Events Center in Farmington,” said a Facebook post from Davis County Health Department.

Parental consent is required for anyone under the age of 18 to receive a COVID-19 vaccine at the Davis County clinic. For those 12 to 15 years of age, a parent or guardian will need to be present at the appointment. For those 16 to 17 years of age, a signature from a parent or guardian is required on the vaccine appointment form.

Clinic hours are 10 a.m. to 8 p.m. on Thursday and Friday and 10 a.m. to 5 p.m. on Saturday.
